Short-term disability forms, FMLA paperwork and mental-health functional assessments completed for a flat $150 fee.
- Flat $150 per request, paid directly — insurance does not cover form completion.
- Available to established patients whose records support the request.
- Includes short-term disability documentation, FMLA certification and functional assessments.
